**Another patch code release

During RB20/CA18 ECU coding for USB consult I have found some significant bugs in the identification code. This will require a matched release of nistune and a new revision of patch code for R31/Z31
the problems are
- missing the response byte header (2 bytes)
- not finishing patch routine cleanup before returning to interrupt handler
- moved streamindex into range with other variables
